ATHLETIC ON-LINE FORMS

Dear Parents of a GISD Student-Athlete:

In an effort to stream line athletic paper work, Graham ISD is offering online forms. It is mandatory that all student-athletes fill out the appropriate UIL and GISD paper work before they will be allowed to practice or try out in athletics.  This website is designed to stream line the process, allowing for less shuffling of papers.

FOR THE 2020-2021 SCHOOL YEAR ONLY THE UIL WILL NOT REQUIRE A NEW PRE-PARTICIPATION FORM FORM FOR ANY STUDENT WHO PREVIOUSLY HAD AN ACTIVE FORM.  IF YOUR STUDENT WAS ELIGIBLE TO PARTICIPATE LAST YEAR THEY WILL NOT NEED A NEW PHYSICAL TO PARTICIPATE THIS YEAR.  YOU WILL ONLY NEED TO COMPLETE THE ONLINE (RANK ONE) FORMS.  ANY STUDENT WHO HAS DID NOT HAVE A PHYSICAL THAT ALLOWED THEM TO PARTICIPATE LAST YEAR WILL NEED ONE FOR THIS YEAR.  THIS WILL APPLY TO ALL SEVENTH GRADE ATHLETES AND MANY INCOMING MARCHING BAND PARTICIPANTS AT THE HIGH SCHOOL.  FOR MORE INFORMATION E-MAIL GRAHAM HEAD ATHLETIC TRAINER MIKE CARROLL AT mike.carroll@grahamisd.com

The actual hard copy of the Pre-Participation Physical must still be turned into the Athletic Trainer on campus. This piece of paper must be signed by the physician, parent, and student-athlete. This form will be given to your athlete or can be printed out from the online forms in Rankone.
