Action Needed: SB 12 Consent for Physical and Mental Health Services

Posted Date: 9/02/25 (4:43 PM)

Dear GHS Families,

As required by SB 12, passed during the 89th Legislative session in Texas, we are requesting your annual consent for your child to receive routine school-based physical and mental health-related services from qualified school staff, including nurses, counselors, and other school personnel, as needed.

This consent supports timely and equitable access to care that promotes student safety, wellness, and readiness to learn. Consent is valid for the entire school year unless you choose to revoke it in writing.

Please complete the attached form by:  September 5, 2025

At some point, Graham ISD may be required to send an amendment or additional consent form.  Currently, the Texas Legislature has passed Senate Bill 12 but districts do not have guidance on implementation. That guidance will come to us in the form of Commissioner's Rules for Senate Bill 12, at a future date.

Thank you for your ongoing support of Graham ISD.
